The concurrent use of many medications. In older adults it raises the risk of falls, confusion, drug interactions and adverse reactions. Build a complete medication list, including over-the-counter and herbal products, consider criteria for potentially inappropriate drugs in older adults (e.g., Beers), and review unnecessary drugs with the prescriber.
