Anticoagulants
Topic: Pharmacology
Drugs that prevent clots from forming or growing: heparins, warfarin and direct oral anticoagulants. The main risk is bleeding: watch gums, urine, stool and bruising, and monitor aPTT or anti-Xa with unfractionated heparin, platelets with any heparin and INR with warfarin. Antidotes are protamine for heparin and vitamin K for warfarin. Teach clients on warfarin to keep their vitamin K intake consistent.
