Transferring responsibility for a client, and the information about them, from one clinician to another at a shift change or transfer. Because communication failures are a leading cause of adverse events, use a structured tool such as SBAR or I-PASS, ideally at the bedside and with the client involved. Always pass on pending tasks, risks and recent changes, and give the receiver a chance to ask questions.
