Informed consent

Permission given voluntarily by a client after being told, in terms they understand, the purpose, benefits, risks and alternatives of a proposed procedure. Explaining is the responsibility of the provider performing it; the nurse confirms that the client has been informed, understands and is not sedated, witnesses the signature and tells the provider if understanding is lacking. Consent can be withdrawn at any time.
