Incident reporting

Reporting events that harmed or could have harmed a client, such as medication errors, falls and near misses, to the facility's safety reporting system. The aim is not to blame but to find system problems and prevent recurrence; in Türkiye the Ministry of Health's Safety Reporting System (GRS) is used. Report objectively; the incident report itself is not placed in the medical record, which records only the event and the care given.
