Topic: Community health
A visit in which the nurse assesses and cares for individuals and families in their own homes, for example home health care for chronically ill clients and their caregivers, or ongoing care that helps prevent the worsening or recurrence of disease. Provide teaching and care, involve the family and link them with community resources for follow-up.
