Pain assessment
Topic: Fundamentals
A systematic inquiry into pain location, intensity, quality, onset, duration and aggravating or relieving factors, often guided by mnemonics such as PQRST or OLDCARTS. Pain is what the client says it is: use numeric or faces scales for intensity and behavioral scales for clients who cannot self-report. Reassess and document after every intervention.
