Localized damage to skin and underlying tissue, usually over a bony prominence or under a medical or other device, from prolonged pressure or pressure combined with friction and shear. It is staged from 1 (nonblanchable redness of intact skin) to 4 (exposed muscle, bone or tendon), plus unstageable and deep tissue injury. Prevent it with regular repositioning, pressure-redistributing surfaces, checking the skin under devices, clean dry skin and adequate nutrition.
