Nursing diagnosis

Topic: Fundamentals

A nurse's clinical judgment about how a person, family or community responds to health problems or life processes. Unlike a medical diagnosis, it names the human response rather than the disease, and it guides interventions nurses carry out independently. It is often written in PES format: problem, etiology and signs/symptoms.
