Braden Scale
Topic: Fundamentals
A tool that predicts pressure-injury risk across six subscales: sensory perception, moisture, activity, mobility, nutrition, and friction and shear. Totals range from 6 to 23; the lower the score, the higher the risk, with 18 or below generally indicating risk. Score on admission and whenever the condition changes, and plan prevention from the result.
