Dehydration
Topic: Medical-surgical
A fluid deficit that develops when the body loses more fluid than it takes in. Thirst, dry mucous membranes, poor skin turgor, scant dark urine, tachycardia and orthostatic hypotension are typical; confusion in older adults and a sunken fontanelle in infants are important signs. Monitor intake and output, daily weight and electrolytes, and give oral or IV fluid replacement as ordered.
