Suicide risk assessment

Topic: Mental health

A systematic assessment of suicidal thoughts, plan, access to means, previous attempts and protective factors. Asking directly ("Are you thinking about killing yourself?") does not increase risk; it gives the client a chance to talk. When risk is high, do not leave the client alone, remove items that could be used for harm, maintain close observation and build a safety plan together.
