Discharge teaching

Teaching that prepares the client and carers to manage safely at home: the diagnosis in plain words, the purpose, timing and main side effects of each medicine, warning signs and when to seek help, diet and activity, wound or device care, follow-up appointments and whom to call. It starts early in the stay, not on the day of discharge; confirm understanding with teach-back and document it.
